Why Getting Assessed Is the First Real Step

If you've been trying to conceive for over a year with regular, unprotected intercourse and it hasn't happened yet, it's worth getting a proper fertility assessment rather than waiting and wondering. The reassuring part is that infertility is, in most cases, manageable once the underlying cause is understood, and that understanding starts with the right set of tests.

When you visit Rekha Sagar Fertility Centre in Karimnagar for the first time, we take the time to go through your complete history: your menstrual pattern, your medical and surgical background, and any previous fertility treatments. If you've had earlier evaluations elsewhere, do bring those reports along; they help us avoid repeating tests unnecessarily and give us a clearer starting point.

Checking Your Ovarian Reserve

One of the first things we look at is your ovarian reserve, essentially, how many eggs remain and how well they're likely to respond to treatment. A few simple blood tests give us this picture:

  • AMH (Anti-Müllerian Hormone): Produced by cells within the ovarian follicles, AMH levels give a fairly reliable estimate of your remaining egg count. It can be tested on any day of your cycle. Higher levels generally point to a larger reserve, while levels naturally decline with age.
  • FSH (Follicle Stimulating Hormone): Tested on day 2 or 3 of your cycle, FSH works the opposite way to AMH: high levels usually signal a lower ovarian reserve, while lower levels are a good sign.
  • Estradiol (E2): Checked alongside FSH, unusually high estradiol levels can hint at problems with either egg quantity or quality.
  • AFC (Antral Follicle Count): Considered one of the most reliable indicators, this involves a vaginal ultrasound to physically count the small follicles in the ovaries. A higher count generally reflects a healthier reserve.

Beyond Blood Tests: Other Diagnostic Tools We Use

Ovarian reserve results alone don't tell the whole story, so depending on your case, we may also recommend:

  • >BBT charting: Tracking your basal body temperature at home to spot the temperature rise that signals ovulation, helping identify your most fertile days.
  • Postcoital test: A sample of cervical mucus taken a few hours after intercourse to see how well sperm are surviving and moving within it.
  • Transvaginal ultrasound: A detailed, internal scan that lets us examine the uterus and ovaries closely for fibroids or structural issues that might be affecting conception.
  • Hysterosalpingogram (HSG): Performed between day 6 and 13 of your cycle, this X-ray based test uses a dye to check whether your fallopian tubes are open or blocked.
  • Hysteroscopy: Reserved for cases where the HSG suggests a blockage or abnormality, this involves a thin camera passed through the vagina to directly view the inside of the uterus.

What Happens Next

Not every patient needs every single test on this list; your doctor at our Karimnagar centre will decide what's relevant based on your history and initial findings. And in some cases, even after a thorough work-up, no obvious cause is found; that's more common than you might think, and it doesn't mean treatment can't move forward successfully.
